I presume you have already extracted the game folder.
Start by navigating to 'bin' within the game folder. After that, click 'Minetest' the application. Its hard to miss.
Once you have done that, 2 windows should pop up after a few seconds. One is the command log, and whatever. Don't touch this window and you will be fine. Click the window that is the actual game, and click the 'Singleplayer' tab if you aren't already there. Click the grey 'New' button, type in a name for your world, skip all the other thing and click create. Now from the list of worlds (there should be only one) click the one you just named, and click "Play". Give it a minute or two to load data and chunks, and there you go. You are now in the game, press I for your inventory and W, A, S, and D, to move, hold left mouse to mine blocks, and right click to place the block your holding. If you are familiar with minecraft, crafting and simple operations shouldn't be hard.
That is how you start a game.
