I have habitat, farming, farming_plus, and nature mods installed and didn't know if those were known to cause problems.
Do you have legacy installed? I removed legacy because I wasn't using an old map, and didn't think I needed legacy. But legacy has three important aliases in it that interact with the C++. Two control the grass growth and death, and the other specifies which node should be treated as a sapling, and grow into a tree. Some of the other aliases may be important as well, I didn't do any major testing.

I've noticed that too. It doesn't tend to matter much in open areas, which is where I usually see it, but it just happened in a walled-in base, where there's no sun to regrow the grass.
It's just a guess, but could that have to do with loaded chunks? I know trees grow flat sometimes, if pressed against a line between map chunks. Maybe the default:dirt_with_grass reverts to default:dirt because the air above it isn't loaded. It would explain why it only happens at certain elevations.
