What are protocol version bumps and how do they work?

Is this something that is done for every network-affecting pull/commit? Once per release?

What kind of network changes trigger a protocol bump?

Do pull-requesters need to do this, or just the puller?

Where would I go to bump the protocol version?
Every time a mod API is left undocumented, a koala dies.

I've read it but it doesn't say anything about protocol bumps or versioning, other than some compatibility things between pre- and post- version 25.
