FOSS gamedev and creative worlds

I am looking for a Teleportation mod that allows you to create a "portal" or "doorway" that allows you to go though and end up in a completely different place (a place chosen by the person who created the portal or doorway). Has anybody seen any? I tried a search but nothing good came up.
