My vote - the one mod that I put globally into every game/world without hesitation is "Immersive Sounds" aka "Ambience"
http://minetest.net/forum/viewtopic.php?id=2807

Honorable mentions (that are also put in globally):
(One of the criteria I have for these is that, if removed, they wouldn't change or negatively affect the world that I used them to develop with.)
* Worldedit and Mutlinode - These two mods are essential for constructing anything more than a 9x9 hut.
* Bookmarks - If you explore or develop over a vast expanses this mod really comes in handy.
* Walking_Light - Usefull for exploring underground and walks at night.
* Watergone - Very useful when developing underwater structures and cleaning up lava spills.

I've already voted for carts, but some more honorable mentions, animated_fluids, walking_light, and growing_cactus/trees These should all be part of the game.

Stairs-plus, more blocks, more ores should also be in the default game. They're just too useful not to include, and they're quite lightweight too (no CPU-hogging or laggy code, etc).
You might like some of my stuff:

I Think...

The diamond mod should be on here... but I do like more ores alot. I don't get the copper rail though... I might need a copper cart??? I don't know, maybe I should ask Calinou.

I must agree that...
The more ores mod is real good, and I love finding minthril on the bottom of a server and not being able to get it :P But more ores is definitely one of my favorites.

I vote for...

I vote for RealTest.

Why? Because it is the most awesome overlooked Minetest-c55 mod. It adds a ton of new minerals to Minetest-c55, they are generated in a more intelligent way than in default and moreores. Oh, and there are LOTS of possibilities to smelt the minerals and create awesome stuff with it.
I'm creating MineClone 2, a Minecraft clone for Minetest.
I made the Help modpack, adding in-game help to Minetest.
